1. ACCEPTANCE OF TERMS
The following terms and conditions (collectively the “TOS”) apply to your use of validatehealthcard.com (“Site”, “we”, “our”, “us”). By using this Site, you accept and agree to be bound by the TOS herein. If the user (“you”) does not agree with the TOS, you should not access or use this Site.
We may modify the TOS from time to time without notice to you. After such change, you will be bound by the modified TOS. It is your responsibility to read and review the TOS periodically. If you do not agree to the changes made to the TOS then you must stop using the Site.
2. DESCRIPTION OF SERVICE
We provide users with an online health card validation Service (“Service”) to help improve accuracy and efficiency in your health care clinic. Included in this Service is the ability to access Ministry Of Health and Long-Term Care databases to retrieve patient information online in your web browser.
The Service does not provide medical, financial, legal, or other miscellaneous advice. All information on this Site is provided “AS-IS” and is solely for informational purposes. Validation checks eligibility and the health card number but does not guarantee payment of the claim since payment is dependent on additional factors. It also does not provide corrected or updated health card numbers or version codes. Therefore, the user assumes the entire risk with respect to the use of this information. The user must check the correctness of the data at their own end before submitting OHIP claims for reimbursement. Since data obtained is from the Ministry Of Health and Long Term Care databases, we cannot guarantee that the information is accurate, complete or up-to-date. We assume no responsibility for inaccurate content. What you decide to do with the information found on our Site is solely your responsibility and we are not liable for any of the actions you take as a result of using our Site.
The use of this Site is at your risk. We cannot guarantee that our Site will be available at all times and that they will be free of viruses or other harmful components. You are responsible for obtaining access to the Site and any of the fees associated with using the Site.
3. SUBSCRIBER FEES
All fees will be paid at the commencement of the Service.
Monthly subscribers to ValidateHealthCard.com will pay the Monthly Subscription Fee on a monthly basis. The initial Service period of the Monthly Subscription is one month. The Service will auto-renew for subsequent one-month periods. The monthly subscription will begin on the date that the user pays for the Service.
The subscriber will be limited to a number of validations per month based on their subscription plan. The Basic Plan allows you to validate up to 100 health cards per month. The Premium Plan allows you to validate up to 500 health cards per month. The Ultimate Plan allows you to validate up to 1,500 validations per month. When the maximum number of validations in the plan is reached, the Service will not validate anymore health cards until the next billing cycle. The validation counter will reset back to zero at the beginning of each billing cycle.
The subscriber may upgrade or downgrade the plan at any time. If the subscriber decides to change the plan, the validation counter will be reset to zero and the subscriber will be charged the new fee of the plan selected. All unused validations in the current plan will be forfeited. It is the sole responsibility of the subscriber to make changes to the plan prior to the next billing cycle.
Following the initial Service period, ValidateHealthCard.com reserves the right to increase the Subscription Service Fee at any time with 30 days of notice to the subscriber. The subscriber authorizes ValidateHealthCard.com to automatically charge, including any fee increases, the subscription Fee based on the payment method chosen and on the subscription plan selected.
4. SUBSCRIBER TERMINATION
The subscription fees will auto-renew every month, from the time you subscribed, at the current published subscription rates until otherwise terminated. If terminated, the Service will cease immediately. After that point, the subscriber’s status will automatically revert to non-subscriber.
ValidateHealthCard.com reserves the right to immediately suspend or terminate a subscription without notice and without liability (a) in the event that the subscriber fails to pay any amount due to ValidateHealthCard.com (b) any necessary reason for ValidateHealthCard.com to perform its obligations and becomes unavailable and/or (c) in the event of any conduct by the subscriber which ValidateHealthCard.com, in its sole discretion, considers to be unacceptable, or in the event of any breach by the subscriber to this TOS. ValidateHealthCard.com may discontinue any of our subscription products at any time.
5. REFUNDS
ValidateHealthCard.com is a pay-as-you-go Service. Consequently, we do not issue refunds, regardless of the reason for cancellation.
6. PRIVACY POLICY
Any information we collect about you is subject to our privacy policy. For more information, see our Privacy Policy.
7. NO COMMERCIAL USE
You agree not to reproduce, duplicate, copy, sell, trade, commercially exploit, or create derivative works from any content contained within this Site. This includes using the information found on our Site to further any commercial purpose through affiliate programs.
8. THIRD PARTY
This Site includes hyperlinks to other sites or resources. You acknowledge and agree that we are not responsible or liable, directly or indirectly, for any damage or loss caused in connection with these third party sites.
9. AVAILABILITY
The Ministry has scheduled downtime for maintenance every second Sunday of the month between 5:00 a.m. and 8:00 a.m. for their system. Our Service will be unavailable during this time.
10. AFFILIATE
We work in partnership with the Ministry of Health and Long-Term Care but do not have any financial interest in the department. You, the user, accepts responsibility for maintaining an active GoSecure ID and GoSecure password in order to use this Site.
